How do I request a new online form?

Email your request to webmaster@southalabama.edu. Make sure to include all form fields and who the form submission notifications should be emailed to.

Are there any restrictions of what type of information the form can collect?

Personally identifiable information like SSN, DOB or financial information like bank account numbers and routing numbers cannot be transmitted via online forms.

Can forms send notifications to multiple recipients?

Yes

Can the forms collect attachments?

Yes, forms can collect images and documents.

Can Web Services help migrate an existing PDF form?

Yes, include the URL of the PDF if it has been uploaded or attach the PDF to your request.
